Documents Required for Applying for a Replacement Work Permit

There are two cases in which you may need to apply for a replacement work permit:

1. In Case of Loss

  • Application form ตท.4 Download here

  • Police report confirming the loss of the work permit

  • Three (3) passport-sized photos (3x4 cm, taken within the last six months)

  • Passport or residence certificate and alien identification card (original and copy)

  • Copy of work permit (if available)

  • Power of attorney with a 10-baht stamp and a copy of the authorized person's ID card (if applying through a representative)

  • Fees: Application fee (100 baht) + Replacement permit fee (150 baht)

2. In Case of Material Damage

  • Original damaged work permit and a copy

  • Three (3) passport-sized photos (3x4 cm, taken within the last six months)

  • Power of attorney with a 10-baht stamp and a copy of the attorney's ID card (if applying through a representative)

  • Fees: Application fee (100 baht) + Duplicate permit fee (150 baht)

Procedures for Applying for a Replacement Work Permit

Follow these steps to apply for a replacement work permit:

Step 1: Report the Loss

Report the loss of your work permit at the local police station and ensure the report includes the lost work permit number.

Step 2: Prepare the Required Documents

Gather all the necessary documents for the replacement application.

Step 3: Submit the Application

Visit the Department of Employment in your work area to submit your application.

Step 4: Verification and Issuance

Once officials verify that all details and documents are correct, and a police report has been provided (for lost permit), they will issue a replacement work permit.

  • Notify authorities within 30 days if your work period is extended under the Investment Promotion Act or other laws. (Failure to do so may result in a fine of up to 500 baht.)

  • Working without a valid work permit is illegal and may result in imprisonment of up to 5 years, a fine ranging from 2,000 to 100,000 baht, or both.
